Most enduros in the US start a row of racers each minute, with each row having 4 or 5 riders. In the above picture, the "45" stands for minute 45 (or row 45.) The letter simply distinguishes that rider from the others on the same row.

Oops, you asked about the front fender. That's the score sheet. When a rider comes through a checkpoint, the time or "minute" is marked on the scorecard. The difference between that arrival time and the racer's number determines how late or early the rider made that check.
